Thanks to you and other generous donors, Form5 Prosthetics can continue offering its workshops and internships in central Ohio and provide life-changing devices to those in need.

Form5 Prosthetics was founded in 2017 to equip individuals with limb differences with the confidence, resources, and support to accomplish anything they can imagine. A 2023 Columbus Foundation 5 Nonprofit to Watch, the organization cultivates a community of individuals with limb differences through its Nub Club of Ohio while providing experiential learning opportunities for high school and college students interested in engineering, design, and medical careers.
